Although this charm is definitely sterling silver, it is much, much, much too small. It would definitely be more attractive if it was bigger. It is about the size of a green pea. If worn with other bracelets, it will be overlooked.
I purchased a sterling silver medical alert bracelet which is verylarge and at the end of this bracelet I added this small charm. I was going to have "diabetes" engraved on the back of the larger medical alert bracelet but decided it would be better to add the charm to it. I also have asthma so I didn't want to engraved diabetes on it. It also may be a little weird to wear a charm bracelet that identifies all your illnesses but in today's world,one has to be prepared for anything and everything.

An adorable, but too small pendant, even for a charm.
If this charm is meant to inform anybody of a medical situation, it would have to fail. Not very suitable as diabetic ID/ medical ID.
